Features · Forms and intake

The intake packet is done
before they walk in.

Build a form once, send the whole packet into the member's thread, and review what comes back in the same queue as everything else. Signed answers stay the way they were signed.

How it works

Built once. Answered at home. Reviewed in your queue.

Members fill in forms where they already talk to you. The answers go to the people who handle the rest of their requests.

  1. 01

    Build it once

Lay out the form, add the logic and scores it needs, and preview it at phone, tablet, and desktop widths. It cannot be published while its logic or formulas are broken.

  2. 02

    Send the packet

Put registration, history, consents, and assessments in one ordered packet. It arrives as a card in the member's thread, each item pinned to the version you sent.

  3. 03

    They finish it at home

Answers save as they go, so a member can stop and come back. Details you already have are filled in and labeled.

  4. 04

    Review it in the queue

A completed form becomes review work in the same staff queue as everything else. Ask for a clarification, countersign it, or lock it.

The builder

Lay out the form the way you want it read.

Drag fields onto the page and nest them in sections and pages. Undo and redo work the way you expect, and every change autosaves, so a half-built form is never lost.

Preview it at phone, tablet, and desktop widths before a member sees it. Start from a blank page, a template, or a copy of a form you already use, and keep the library in folders with your favorites at the top.

Fields and logic

Every common field, and logic that never guesses.

Text, names, and addresses
The fields every registration form starts with.
Numbers and currency
Numeric answers and amounts, ready to use in calculations.
Choices
Single and multiple choice, and searchable dropdowns for long lists.
Dates and times
Birth dates, symptom onset, last visits, and preferred times.
Matrices and repeating tables
Rate several items on one scale, or let the member add a row for each medication, allergy, or relative.
Show and hide
Fields, sections, and whole pages appear on any combination of answers: all of these, any of these, and groups within groups.
Hidden answers are dropped
When a field is hidden, its answer is removed from the submission. It is not kept out of sight.
Calculations and scores
Totals, dates, and scoring formulas. A missing answer shows as missing, never as zero.
Checked before publishing
A form with broken logic or a formula that does not work cannot be published until it is fixed.

Where they arrive

Sent into the conversation they already have with you.

There is no separate link to lose. A form or packet arrives as a card in the member's thread, next to your message about it, and waits in their Forms view until it is done.

Answers save as they go, so a member can start at home and finish in the parking lot. Details you already have, like an address or a pharmacy, are filled in and labeled as coming from their record. They can change them where you allow it.

Signatures and review

Signed answers stay the way they were signed.

A packet is an ordered set of forms and documents, and each item is pinned to the version you sent. Editing the form later does not change what the member is answering.

The member, or their representative, signs electronically. Signed answers are locked. A correction is an amendment with a name and a time on it, so nothing is changed quietly after the fact.

Every completed form lands in the same staff queue as refills and scheduling. From there your team can request a clarification, countersign, or lock it.

At the front desk

For the forms filled in at the office, and the reports after.

Kiosk mode
Hand a tablet across the desk. Kiosk mode resets between patients, so the next person starts on a clean screen.
Paper forms, attributed
When a form comes in on paper, staff enter the answers and the entry is recorded under their name.
Staff-only fields
Fields for your team's notes stay off the member's copy of the form.
Anonymous surveys
Collect feedback without tying the answers to a person, when that is what you want.
PDF copies
A PDF of the completed form for the chart.
CSV export
Export responses to CSV for your own reporting.
