Your EMR
Your EMR runs the chart.
We run the relationship.
You are not switching systems. Practice Portal sits beside the EMR, reads what each member is allowed to see, and hands your staff finished work instead of raw messages. Clinicians can draft notes with the scribe and send them to the EMR to sign.

Supported integrations
Elation Health and Cerbo.
Each practice connects one EMR. Changes in the chart show up in the member's app without anyone re-keying them. An owner or administrator enters the EMR's API credentials in Settings, and they are checked against the EMR before they are saved.
- Elation Health
- Uses Elation's API and webhook subscriptions. Connect your Elation sandbox, see your own data flow, then switch to production when you are ready.
- Cerbo
- Uses a dedicated Cerbo API user. Changes are picked up every five minutes, with a daily full refresh; an optional Cerbo webhook brings patient updates in sooner.
- Staleness is visible
- If the connection lapses, dependent actions pause and the request stays owned. Nothing pretends to be current.
Other EMRs
We add an integration when practices ask for one, and we do not list it before it works. Tell us what you run on the early-access form. Until then, practices on any of these run Practice Portal with staff-entered records.
- Hint Health
- Healthie
- Atlas MD
Some EMR vendors charge the practice for API access. Where that applies, it is a fee from your EMR, not from us; we will tell you what to expect for yours before you connect.
What flows in
Read from the chart.
Each of these is a per-practice switch. Turn off anything you do not want members to see.
- Patients
- Demographics and contact details. Matched to the member account once; never re-entered.
- Appointments
- Upcoming and past visits, with type, clinician, and modality.
- Medications
- The active list the member picks from when requesting a refill.
- Allergies and intolerances
- Shown in the chart view; available to staff reviewing a refill. Cerbo's API does not expose drug intolerances.
- Lab results
- Panels and individual results with flags, as they post.
- Vitals
- Individual readings and trends over time. From Cerbo: blood pressure, height, and weight.
- Visit notes
- Notes the practice has marked as member-visible.
- Clinical documents
- Letters, referrals, and other documents on file.
- Immunizations
- Complete record with dates.
- Problems
- Conditions from the chart's history, shown to members when you allow.
- Family history and photos
- Imported for the care team only. Members never see them. Cerbo's API does not expose family history.
What flows back
One thing, and only when a clinician sends it.
- Scribe notes, when a clinician sends one
- After signing a scribe note in Practice Portal, the clinician can send it to the EMR under their mapped account: an unsigned, confidential SOAP draft in Elation, or an unsigned encounter in Cerbo. It is sent once, and they sign it in the EMR. Practice Portal never signs anything in your EMR.
- Nothing else
- No appointments, medications, allergies, or signed notes. Bookings made in Practice Portal live on its own calendar. Refill and referral requests become tasks for your staff, who act in the EMR as they always have.
Recovery
When a sync fails, you find out. It does not quietly go stale.
- Scheduled retries
- Sync jobs that stall are retried automatically, up to three times, before anyone has to look.
- Daily backfill
- Elation subscriptions are refreshed and every patient is re-synced on a rolling daily schedule; Cerbo is polled for changes every five minutes and fully refreshed daily. A missed update does not become a stale chart.
- Owner controls
- Owners see failed jobs and records with a plain-language reason, and can retry a job or reprocess a single record.
- Nothing silently dropped
- If the EMR does not return something it should have, the record stays marked failed for review instead of disappearing.
